This form records your explicit consent to take part in the Eliwana Heat & Fatigue Study. It explains what data will be collected, how it will be used, and your rights. Please read it carefully and ask any questions you need.THE STUDYThe Eliwana Heat & Fatigue Study is a voluntary, observational study using a wearable device to collect physiological and environmental data during your shifts at Fortescue's Eliwana site. Your data will be used to improve work practice and safety on site, including the testing and refinement of heat and fatigue detection models.WHAT DATA IS COLLECTED- Health and biometric data: measured continuously by the wearable device while you wear it on shift.
- Demographic data: your name, employee or contractor ID, age, sex, role or work group, and the testing group you are part of.
- Environmental data: site conditions during your shift, such as temperature and humidity.WHAT DATA IS NOT COLLECTED- The device has no microphone and does not record audio.
- The device has no camera and does not capture images or video.
- The wearable device does not have GPS and does not track your location.
- The app may occasionally check your phone's GPS to confirm you are at Eliwana, so your data is recorded against the correct study. The app does not track your movement around the site.WHO HAS ACCESS TO YOUR DATA- Omni Biotech research, engineering and support staff have access to your data, under strict access controls.
- Fortescue and their Contractors receive only aggregated and de-identified reporting from the study. No individual worker data is shared with Fortescue, your employer or your supervisor.
- No third parties have access to your data.WHERE YOUR DATA IS STOREDAll data is stored in Australia (Sydney) in secure AWS data centres. Data is encrypted in transit and at rest.HOW LONG WE KEEP YOUR DATA- Raw physiological data: retained for 2 years after the end of the study.
- De-identified or aggregated data: may be retained indefinitely. Once de-identified, this data can no longer be linked back to you.PARTICIPATION- Your participation is entirely voluntary.
- Your decision to take part, or not to take part, has no impact on your employment, shifts, pay or site access.
- You can withdraw at any time, for any reason.MORE INFORMATIONYou can, at any time:- Access the data we hold about you.
- Correct any inaccurate information.
- Withdraw your consent. Withdrawal stops further collection of your data from the point of withdrawal.
