This form records your consent to take part in the Eliwana Heat & Fatigue Study. It explains what data will be collected, how it will be used, and your rights. Please read it carefully and ask any questions you need.THE STUDYThe Eliwana Heat & Fatigue Study is a voluntary, observational study run by Fortescue.It uses wearable devices to collect health metrics like heart rate, skin temperature and steps during shifts.The aim is to build a picture of how heat and fatigue impact workers across different roles, environments and work types in real on-site conditions.Participation is:- Voluntary: it's your choice to participate and you can withdraw at any time.
- Anonymous: all data is de-identified and only you can access your data.
- Time-bound: you're only expected to wear the band for one 2-week swing.All data is de-identified before reporting so that no individual is identified in any study output. Your individual data is not seen by your supervisor, your employer, or Fortescue.YOUR DATAWho has access?To manage the study and provide support, Omni Biotech staff can access your data, under strict controls. Fortescue and contractors receive only aggregated and de-identified reporting from the study. No individual worker data is shared with Fortescue or your employer.What data is collected?Only seen by Omni:- Demographic: name, work ID, age, height, weight, gender.De-identified for reporting:- Health: collected by wearable on shift.
- Team: role, work group, and testing group.What data is not collected?The device does not record audio, capture images or video, or track your location. Omni's app may occasionally check your phone's GPS to confirm you're at Eliwana, so your data is recorded against the correct study.How your data is storedAll data is stored in Australia and encrypted in transit and at rest. Raw physiological data may be retained for 2 years after the end of the study. De-identified or aggregated data may be retained indefinitely.CONSENTBy completing this form, I acknowledge that I:- have read and understood this form and been able to ask questions about anything I didn't understand.
